How to Submit an IRB Modification, Status Check, and Continuing Review
Submitting an IRB Modification
A modification should be submitted to the Cal Poly IRB when changes need to be implemented to an existing, active project. Changes that should be submitted for review include but are not limited to:
- Change in Personnel
- Change in Data Collection Procedures
- Change in Data Collection Instruments
- Change in Funding
- Change in Recruitment Methods
- Change in Informed Consent Forms
- Change in Location
- Change in Study Population

Submitting a Continuing Review or Status Check
A Continuing Review or Status Check should be submitted to Cal Poly IRB when a project is near expiration (projects expire one year minus a day from the approval date). Continuing Reviews and Status Checks both renew the project for another year but are required for different types of projects:
Status Checks - The Status Check form is used for projects that have been determined 'Minimal' by the IRB office.
Continuing Reviews - The Continuing Review form is used for projects that have been to be determined 'Expedited' or 'Full Board' by the IRB office.
Please Note: IRBManager sends emails when projects are nearing their expiration date. However, an error in IRBManager has led to some projects not receiving these emails or not having an expiration date in the system. If you are looking to renew your project and did not receive an email, you will need to go into your project in IRBManager, click on 'Start xForm' on the left side of the webpage, and then select either the Status Check or Continuing Review form
